Mobile Intensive Support Rehabilitation-Style Day Program, delivered in your community.

Roadmaps is a free program that runs for two sessions a week for six weeks. We provide intensive rehabilitation support which includes individual and group counselling, information and referrals to other support services. The program includes education sessions focusing on harm reduction and overdose prevention.

Clients entering the program will receive a comprehensive assessment, including a mental health assessment.

Our program

Our team work with you to develop a care plan to start things off. Over the course of the six week program, the Roadmaps counsellors / case managers help you get back on track with:

  • Individual assessment
  • Education and awareness
  • Setting goals and developing strategies
  • Harm reduction
  • Relapse prevention
  • Access to supports and community.

Roadmaps is a free and voluntary program that runs for 2 sessions per week over 6 weeks, with follow up.

Who can access the program?

Roadmaps is a program that supports adults aged over 18 from rural and regional NSW. You can self-refer or be referred by a third party.

How do I get a referral?

You don’t need a GP referral. You can self-refer through our website here. If you have limited access to the internet or would prefer to speak to us to request a referral, please phone the Contact Team on 1300 727 957.

We also accept online provider referrals from other health professionals, services and community and government organisations or they can fax referrals to 07 5546 8223.
