Central West Mental Health Psychosocial Support

At Lives Lived Well, we understand that people with severe mental illness may require additional support in their daily lives. That’s why we’re proud to partner with RHealth to offer free psychosocial coordination and brokerage services in the Central West.

Thanks to funding provided by Western Queensland PHN under the Commonwealth Psychosocial Support Program, we’re able to provide person-centred and recovery-focused support to individuals ages 16 years and over presenting with complex mental health needs.

Our case coordinators can help you identify and work towards your goals in one of the following key areas:

  • Social skills, friendships and family connections
  • Day to day living skills
  • Financial management and budgeting
  • Finding and maintaining a home
  • Vocational skills and goals
  • Managing physical wellbeing, including exercise
  • Building broader life skills and building capacity to live independently in the community.
