At Lives Lived Well, we’re proud to offer the Registered Nurse Care Coordinator (RNCC) Program, designed to support individuals and health professionals in addressing substance use and related health concerns.
The Nurse Care Coordinator utilises a two-pronged approach:
- Our commitment is to help clients address substance use and improve their overall health by providing case management, service navigation, and health literacy. We also encourage and support medical treatment compliance.
- For health professionals in the Blue Mountains and Lithgow areas, we offer expert guidance and practical assistance in managing patients or clients presenting with substance use challenges.
We understand the complexities involved in supporting this client group, including the significant time and resources required to address multiple health and social concerns. Our Nurse Care Coordinator is here to ease this burden, providing expertise and tailored support to create positive outcome.
What support does the Nurse Care Coordinator provide?
Our Nurse Care Coordinator offers a range of support, including:
- Helping develop care plans
- Assisting with substance use screening
- Offering intensive AOD assessments and referrals to treatment services (if required)
- Facilitating access to health and social resources
- Attending client appointments for additional support
- Supporting clients receiving Opioid Replacement Therapy (ORT)
- Assisting GPs with ambulatory detoxes
- Providing AOD education for health professionals, clients and families
- Supporting clients with Nicotine Replacement Therapy (NRT) in the community.
How to access the program?
You don’t need a GP referral. You can self-refer through our website here. If you have limited access to the internet or would prefer to speak to us to request a referral, please phone the Contact Team on 1300 727 957.
We also accept online provider referrals from other health professionals, health services, and community and government organisations. They can also fax referrals to 07 5546 8223.
How much does it cost?
This service is free for all clients, General Practitioners, nurses, and allied health professionals.

The Nurse Care Coordinator works across the Nepean Blue Mountains Local Health District, covering the Blue Mountains, Lithgow, Hawkesbury, and Penrith. Outreach to health professional practices is also available.
Education sessions are available for clients, families, and health professionals on topics like:

- Harm minimisation strategies
- Alcohol and drug screening
- Opioid and Nicotine Replacement Therapy
- Withdrawal management and residential rehabilitation options.
For clients, there are social groups held at Dianella Cottage run by the Nurse Care Coordinator, mindful art and healthy recovery lifestyle. Groups are run weekly or fortnightly during school terms. Please contact the Nurse Care Coordinator if you wish to join these groups.
Yes, the Nurse Care Coordinator can accompany clients to appointments to provide additional support.
