Sometimes, not all needs can be met by just one service… that’s where we step in

We work with young people between 17 and 29 years who have complex needs arising from problems with alcohol and drugs, as well as mental health issues or a disability.

The Complex Needs Assessment Panel and Integrated Services program provides a coordinated and holistic approach to supporting young people. Our expert panel helps young people achieve positive outcomes in life & health by bringing together a range of services to address their multiple needs.

We can help break down barriers to get young people access to the support they need through integrated case planning and collaborative partnerships.

WHAT DOES IT INVOLVE?

If you have problems across the areas of health, housing, education, employment or legal issues – we can call on the specific government or community services to assist. We will help identify your needs and go in to bat for you to ensure support is provided.

Our friendly staff will talk to you about how we can best help out.

DO I NEED A REFERRAL?

Yes, you will need a case manager from another agency to refer you to our CNAPIS program. Unfortunately, you cannot self refer. However, you do not need a medical referral.

HOW DO I REFER MY CLIENT?

Firstly, some criteria applies. To refer a client, they must:

  • reside in the Gold Coast region
  • be between the ages of 17-29 years
  • have provided informed consent
  • have significant alcohol and other drug use issues/concerns.

While the panel provides clients with added support, the referring agency continues in the role of primary case management.

If you are working with a client with complex needs and feel drugs or alcohol are a problem, please give us a call. Phone 07 5535 4302 or 1300 727 957 or email your query to info@liveslivedwell.org.au. Our friendly staff will talk to you about how we can best help out.

IS THERE A COST?

No, services provided by the CNAPIS team are free of charge.

WHERE ARE WE LOCATED?

CNAPIS is offered from our Gold Coast service which is located at 191 West Burleigh Road, Burleigh Heads.
